Mycoplasma or Ureaplasma Found in Urine: What Does It Mean and Should You Panic?

Imagine: you've taken a test, and a week later the doctor scratches his head in puzzlement and says, 'You have mycoplasma.' Sound familiar? Many people immediately panic, recalling horror stories about infections. But it's not all that scary. Let's figure out what these mycoplasmas and ureaplasmas actually are and why they ended up in your test.

Not Quite Ordinary Bacteria

Mycoplasmas and ureaplasmas are bacteria without a cell wall. Sounds boring, but this very feature makes them so resilient and resistant to most antibiotics. Conventional drugs 'attack' the wall, but here there simply is none. Therefore, they can only be treated with three groups of antibiotics, and even then not always successfully.

These bacteria live on mucous membranes—in the mouth, nose, and genitals. Here's an important point: they can be part of your normal microflora, and you won't even know they exist. But there are four strains that can cause problems: Mycoplasma genitalium, Mycoplasma hominis, Ureaplasma parvum, and Ureaplasma urealyticum.

Men: When Something Goes Wrong

In men, these bacteria more often make themselves known. Symptoms can resemble gonorrhea or chlamydia: pain during urination, itching in the urethra, discharge. Moreover, Mycoplasma genitalium is the second most common cause of urethritis (inflammation of the urethra) after chlamydia. In men who have sex with men, inflammation of the rectum may occur.

Women: Cunning Silent Types

In women, things are trickier. Often these bacteria cause no symptoms at all, so they are less frequently detected. But when they become active, more serious problems can arise: cystitis, pelvic inflammatory disease, endocervicitis (inflammation of the cervix) with discharge and pain. Mycoplasma hominis is associated with bacterial vaginosis.

Worse, studies show that high levels of Mycoplasma hominis in the vagina may be associated with increased HIV shedding in infected women. This means the risk of transmitting the virus to a partner is higher at such a time. But this does not mean that mycoplasma causes HIV—it simply exacerbates the situation.

So What Now?

The main thing is not to panic prematurely. First, mycoplasmas and ureaplasmas often require no treatment at all if there are no symptoms. They just live there and don't bother anyone. Second, if you have complaints—pain, discharge, bladder issues—the doctor should figure out what exactly is causing the inflammation and prescribe an appropriate antibiotic. But be prepared: due to drug resistance, the treatment regimen may be complex and not successful first time.

If you notice unpleasant symptoms in the intimate area or problems with urination, don't postpone a visit to the doctor. Especially if you are in a risk group for STIs. And remember: these bacteria are sexually transmitted, so it's worth discussing examination and treatment with your partner together to avoid passing the infection back and forth.
